The OnSite-OccuHealth Minor Work Permit Process

Here’s what to expect during your visit:

Pre-Appointment Preparation: Ensure the Minor Work Permit Application form (link above) has been largely completed by the employer, parent, and school.
Check-in & Physical Exam: Upon arrival, the minor will undergo a basic physical examination. This typically includes a review of health history, vital signs, and a vision screening to ensure they are medically fit for the work they intend to do.
Physician's Certificate Completion: Our medical staff will complete Section 3 (Physician's Certificate) of the application form after the successful completion of the physical exam.
Payment: The fee for the Minor Work Permit Physical is $85.
Next Steps: Once Section 3 is completed by OnSite-OccuHealth, you will take the entire, partially completed form to the minor's school district. The school district is responsible for issuing the final Minor Work Permit. We provide the necessary physical and medical certification; we do not issue the final permit directly.
