Physician's Certificate Completion: Our medical staff will complete Section 3 (Physician's Certificate) of the application form after the successful completion of the physical exam.
Payment: The fee for the Minor Work Permit Physical is $85.
Next Steps: Once Section 3 is completed by OnSite-OccuHealth, you will take the entire, partially completed form to the minor's school district. The school district is responsible for issuing the final Minor Work Permit. We provide the necessary physical and medical certification; we do not issue the final permit directly.
