OnSite-OccuHealth offers visits in our office or our providers can come to your company for your DOT driver exam.
Our company provides all the necessary paperwork and equipment. Your office provides a small private room for us to set-up and perform the necessary testing.
DOT physical exams are uploaded directly to the FMCSA by our office within 24 hours (driver is still responsible for providing a copy to their BMV).

The $50 set-up charge is in addition to charges for physicals and other services each time our provider visits.
We will come out to your company for as few as 5 drivers or as many as 30 drivers for the $50 charge (Toledo area)
Driver's exams are scheduled every 20-30 minutes.
Save money by scheduling all your drivers on the same days visit.

2 DOT Physicals $200 + 2 Audiograms $100 + Set-up fee $50 = Total $350
Areas served in the Toledo Lucas County region will be charged a $50 set-up fee for that days visit.
Areas served outside Toledo will be charged based on the distance travelled.
