To ensure a smooth and successful visit, please pay close attention to the following:
What to Bring to the Appointment:
What to Bring to the Appointment:
Your Photo ID: Parents or legal guardians, please bring your current state-issued identification/driver's license. Without it, we cannot perform the exam. No ID, no exam – plain and simple.
Proxy Authorization (if applicable): If someone other than the parent or legal guardian is bringing the student, they must have:
A copy of the parent/guardian's photo ID and their phone number.
A signed note from the parent/guardian explicitly granting both the accompanying adult and OnSite-OccuHealth permission to perform the exam.
Glasses/Contact Lenses: If your child wears corrective lenses, they must bring them to the exam. Their vision will be tested, and they may not pass without their eyewear.
Medication List: Please bring a list or a clear photo of all prescription medications and supplements your child is currently taking. This is vital for a comprehensive health assessment.
OHSAA Physical Form: While we provide the examination, please remember to bring the blank OHSAA Preparticipation Physical Evaluation form from your school for us to complete. This ensures all necessary sections are filled out accurately.
