• Onsite-OccuHealth is now offering Preparticipation physical examinations for high school students.

• Physical examinations are in compliance with the Ohio High School Athletic Association.

• Physical exams are $75. Sorry, We do NOT bill insurance on Sports Physicals

• If you are a school, coach, or youth group please contact Andrea at our office. We can arrange to come out to your school if you have multiple students that require testing.

• PARENTS PLEASE BE SURE TO BRING YOUR IDENTIFICATION/DRIVERS LICENSE WITH YOU OR WE CANNOT PERFORM THE EXAM.

• IF YOU ARE SOMEONE OTHER THAN THE PARENT/GUARDIAN THAT IS BRINGING THE CHILD TO THEIR VISIT OUR OFFICE WILL NEED

1. A COPY OF THE PARENT/GUARDIAN PHOTO ID AND THEIR PHONE NUMBER

2. A NOTE FROM THE PARENT/GUARDIAN GIVING YOU AND ONSITE-OCCUHEALTH PERMISSION TO PERFORM THE EXAM.

• IF THE CHILD WEARS GLASSES/CONTACT LENSES THEY MUST BRING THEM TO THE EXAM OR THEY MAY NOT BE ABLE TO PASS

• PLEASE BRING A LIST OR PHOTO OF THE CHILDS MEDICATION IF THEY TAKE PRESCRIPTIONS/SUPPLEMENTS.
